This role will work with our team who are committed to delivering the highest possible standards of care to babies, children and young people making a real difference to those we support and their families, in-house in our Hospice, Chorley, and in the wider community. Main Responsibilities You will work across the four pillars of advanced practice (clinical practice, leadership, education and research) in line with the Health Education England Framework, providing advanced palliative and end-of-life care for our babies, children and young people (CYP). Deliver and manage high-quality, holistic palliative and end-of-life care across hospice, community, hospital and family home settings, working autonomously across organisational and professional boundaries. Provide expert clinical leadership, including advanced assessment and prescribing, with the authority to refer, admit and discharge CYP, while acting as a key expert resource and visible role model to support multidisciplinary teams and families. Provide senior professional development and clinical leadership across the hospice, leading the development of evidence-based practice, local policy, service redesign, clinical governance and quality improvement. Deliver autonomous,, advanced care planning, safeguarding, hospital avoidance and participation in a 24-hour on-call service. Promote education, research, audit and workforce development by mentoring and supervising team members, disseminating learning and research, and fostering a culture of continuous improvement and learning, safety, and regulatory compliance.
