Job Description: We are looking for a proactive and compassionate Care Manager to oversee our domiciliary care services. The Care Manager will be responsible for coordinating care plans, managing a team of care providers, and ensuring the well-being of our clients receiving home-based care. This role requires strong organizational skills, a deep sense of empathy, and a commitment to upholding the highest standards of care.
Key Responsibilities:
1. Develop individualized care plans for clients receiving domiciliary care
2. Supervise and support a team of care providers, ensuring compliance with care plans and regulations
3. Conduct regular assessments of clients’ needs and adjust care plans accordingly
4. Collaborate with healthcare professionals, families, and external agencies to provide comprehensive care
5. Monitor the quality of care provided and implement improvements as needed
6. Maintain accurate records and documentation of care services
Qualifications:
1. Degree in Health & Social Care or a related field
2. Previous experience in a care management role, preferably in domiciliary care
3. Excellent communication and interpersonal skills
4. Ability to lead and motivate a team in a fast-paced environment
5. Knowledge of relevant healthcare regulations and best practices in domiciliary care
