Key Responsibilities
The Role: Leeds Mental Health Outreach Worker
Providing care and support for clients who have Mental Health conditions such as schizophrenia, personality disorder, depression and anxiety.
This support could be in the clients home or out in the community.
Some of whom often present with behaviour that challenges services, self harm and substance misuse.
You will report any safeguarding concerns to the local authority and any relevant professionals who are supporting the client.
You will travel to different client homes within Leeds and you may support from 1-3 clients a day.
You will be flexible and will be able to work a variety of shift patterns including day/evening shifts and sleep overs. You will work alternate weekends, bank holiday's/ any other holidays.
You will be working with clients who need encouragement and motivation to live a meaning full life.
You will support clients to manage their mental health safely. This may include support around their medication, attending appointments, encouragement and motivation around exploring their interests. Maintaining positive relationships.
Supporting and encouraging them with money management skills, cooking and cleaning, improving their physical health, support clients in social/leisure activities within both the home and the community.
You will be in regular communication with other professionals and services who work with our clients.
You will report any safeguarding concerns to the local authority and any relevant professionals who are supporting the client.
Working closely with a talented manager you will ensure the service meets the needs of all individuals it cares for in line with the St Anne's Values.
You will be a team player and will occasionally cover other services in a staff members absence.

1. You will have a caring, confident and well organised personality with the ability to adapt to situations quickly as every day is different.
2. Its desirable that you have experience of working in a care environment with individuals who have a mental health diagnosis
3. Proven track record of team work and working in a face paced environment.
