Band 5 Community Dietitian
Responsibilities:
* Assess, implement and monitor nutritional care plans for adult patients referred to the service.
* Provide services in clinics (face‑to‑face and via phone), home visits, care homes and the ward‑based intermediate care unit.
* Maintain a wide range of dietetic skills, including nutrition support, gastroenterology, liver and pancreatic conditions, diabetes and other long‑term conditions.
* Support appropriate prescribing of nutritional supplements and participate in malnutrition training, including screening and first‑line interventions.
* Train student dietitians during their placements as needed.
