We are a leading Case Management company specialising in the support and rehabilitation of individuals who have suffered personal injuries, including Acquired Brain Injury, Traumatic Brain Injury, Spinal Injury, and other life-altering injuries. Our aim is to support adults with complex injuries to receive the support and rehabilitation required for them to live their best life.
Positions: Case Manager / Associate Case Manager
Coordinate and manage Goal Orientated rehabilitation and support services for clients with personal injuries.
Develop individualised Case Management Plans that are Person Focussed and tailored to each client's needs and goals.
Collaborate with healthcare professionals, therapists, and social workers to ensure cohesive and effective care.
Monitor and evaluate client progress, adjusting Case Management Plans as necessary.
Provide Compassionate support and guidance to clients and their families.
Maintain detailed and accurate records of client interactions and progress.
Advocate for clients' needs and rights within the healthcare system.
Professional Registration - Candidates will hold a professional registration from a Nursing, Occupational Therapy, or Social Work background. Other backgrounds, such as Physiotherapy, may also be considered.
Diligence - Must be detail-oriented and thorough in managing client cases.
Communication Skills - Excellent verbal and written communication skills are essential.
Mental Health Understanding - Strong knowledge of mental health issues and their impact on rehabilitation.
Conflict Management - Ability to manage and resolve conflicts effectively.
Case Management Skills - Proven experience in case management and coordination.
Time Management - Excellent time keeping and time management skills, with the ability to manage several workflows simultaneously
Organisation - The ability to be able to problem solve and use critical thinking to identify issues, mitigate against them and come up with viable solutions
Skills required:
· Minimum of 2 years Case Management experience (essential)
· Knowledge of Community and Statutory Services (essential)
· Knowledge of the Qunote system (desirable)
· Evidence of recent CPD, with the ability to provide documentation of professional learning and development (essential)
