Eden Brown Synergy are currently looking for an experienced Assistant Practitioner to join the Children With Disabilities (CWD) Team for Southend Council.
Duties and Responsibilities:
* Supporting Social Workers with assessments, direct work, home visits, and ongoing case management for children with disabilities.
* Assisting with the development and delivery of Child in Need and Short Break Plans, ensuring needs are clearly identified and interventions are effective.
* Working closely with families, schools, health professionals, and community partners to promote coordinated, child-centred support.
* Providing practical and emotional support to families, helping them navigate services and access appropriate resources.
* Ensuring all case recording is accurate, timely, and compliant with statutory requirements and local safeguarding procedures.
* Contributing to team meetings, multi-agency discussions, and reviews to support effective planning and decision-making.

* Hybrid working
Pay Rate:
* £41.50 p/h Umbrella
Essential Requirements:
* Experience working with children with disabilities or within children's social care
* Strong understanding of safeguarding and statutory guidance
* Ability to work collaboratively within multi-agency settings
* Enhanced DBS
* Excellent communication and organisational skills
