We have an exciting opportunity for a community mental health nurse to join our Haringey Older People Services team based in the Victoria Unit at St. Ann's Hospital. We cover the London borough of Haringey.

As a multi-disciplinary team, we provide needs-led holistic assessment and treatment pathways for older adults and people with mental health and frailty needs. Our service offers seamless pathways between primary and secondary care for individuals with functional mental illness and dementia.

We seek a dynamic nurse passionate about caring for our older adult service users. Transferable skills from other mental health areas are valued and contribute positively to our well-rounded team.

Our team includes Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers, and Mental Health Nurses. We also offer CPD sessions, reflective practice, staff support, supervisions, and training opportunities.

The post holder should demonstrate expert practice in caring for clients with mental health needs, maintain clinical expertise, and take responsibility for risk assessments, mental capacity, best interest assessments, crisis planning, dialog plus, and core assessments, ensuring evidence-based care aligned with NICE guidance.

Flexibility in work locations across Barnet, Camden, Enfield, Haringey, or Islington may be required.

Key responsibilities include:

1. Managing a caseload as Key Worker/Care Manager, ensuring multidisciplinary service delivery and community reintegration.
2. Providing culturally sensitive care considering individual needs.
3. Designing care plans based on risk and psychosocial needs, and participating in discharge planning.
4. Participating in the Duty System, screening referrals, arranging assessments, and liaising with agencies.
5. Maintaining accurate, comprehensive records in line with NMC standards and Trust policies, utilizing IT systems.
