Futures+ is an adult specialist service in East Riding Council. We provide assessments, care & support planning, and reviews for adults with learning disability and/or autism who have high acuity and complexity of needs.
Futures+ is based at County Hall, Beverley. There is an opportunity for hybrid working.
We are a very friendly and extremely supportive team with diverse experiences, knowledge, and skills. We are looking for an experienced social work practitioner, preferably in adult social care, to work alongside our current senior social workers to support the team manager to continue to develop the services.
Due to the demands and challenges to the role, we are looking for a strong team leader with excellent interpersonal and organisational skills who can work under pressure.
You need to be a competent and confident practitioner who can work collaboratively with all stakeholders. If you are a passionate practitioner who are committed to making a positive difference to the lives of others, you will find lots of job satisfaction being a Futures+ senior social worker.
The role can be demanding so you need to be a team player with a can-do attitude.
Within Futures+, we have two hubs, Preparing for Adulthood (PfA) and Community Inclusion (CIH). We are recruiting a part-time 18.5 hours a week senior social worker. PfA Hub supports children aged 14 or above, who have complex needs, preparing for their adulthood. Service users will transfer from PfA hub to CIH hub either when their education ends or when they turn twenty-five years old.
In Futures+, we embed person-centred and strength-based approach when working with our service users, their carers and other professionals to carry out care act assessments, formulate care and support plans, put together panel applications and conduct reviews. We work collaboratively with colleagues from other agencies and contribute towards multi-agency meetings.
You will be expected to share the responsibilities with other senior social workers to oversee duty and support duty officers. In Futures+, we pride ourselves in our strong team spirit. We have an induction program to support new member of staff.
