Job Details The Hospital Social Work Team, East, works with individuals to promote a person-centred approach to hospital discharge planning. The Hospital Social Work Team, East covers East Fife Community Hospitals which include Cameron Hospital, Glenrothes Hospital, Adamson Hospital, Stratheden Hospital and St Andrews Hospital. When required, this team also provides a service to Fife residents in Ninewells Hospital and Perth Royal Infirmary. This post will primarily be based at Fife House and Cameron Hospital with the understanding that depending on demand, other community hospitals may need to be covered. You will work collaboratively with multidisciplinary teams to support people to move from hospital to more appropriate settings. You will carry out personal outcome focused assessments as part of a multi-disciplinary team and work with people aged over 16 years who have complex physical or mental health needs to promote their rights and to achieve better outcomes. Good skills in caseload and performance management are crucial to provide a high-quality service whilst meeting Scottish Government's standards on delayed discharge. An understanding and experience of working in a secondary setting is desirable. This is an exciting opportunity to join a Hospital Social Work Team, East Fife, as a Social Worker. The East Team comprises of a Team Manager, 2 Senior Practitioners, 12 Social Workers and 3 Social Work Assistants. The hospitals which the team covers are; Glenrothes, Cameron, Stratheden, Adamson and St Andrews Hospitals. We also cover Fife residents who are in-patients in Ninewells, Dundee and Perth Royal Infirmary. There is a Hospital Social Work Team, West Fife and we work closely together, providing cover as required. The Social Worker for the East Team will work closely with the multi-disciplinary teams in completing outcome focussed assessments and risk assessments in supporting individuals and their families/carers to facilitate an appropriate and person-centred discharge from hospital. Once discharged from hospital, the Social Worker will undertake six-week reviews before transferring to a community social work team, if appropriate. Currently, adult support and protection investigations are undertaken by community social work teams, although this may change in the future. The appointed Social Worker should have a good knowledge/experience of hospital discharge policies/guidance, the impact of delayed hospital discharge, experience/knowledge of multi-disciplinary working and a genuine interest in working with individuals and their families/carers who require a social work assessment to facilitate a discharge from hospital. The team is a busy team, and the successful candidate will need to manage a work environment that can feel pressured. You will be able to facilitate timely discharges from hospital, minimising any delays. There will be opportunities for personal and professional development. Team meetings and practice development meetings are an integral part of this team. There is an occupational pension available and staff discounts. Lone working is an aspect of this post although most contact will be within a hospital-based environment. The Person A Degree (BA Hons) in Social Work or equivalent is essential, as is registration with the Scottish Social Services Council. Experience in a relevant setting is desirable as is knowledge and understanding of hospital discharge policies, adult support and protection legislation, adults with incapacity and mental health legislation. Ideally, you will have some experience of working within a hospital environment although this is not essential. You are required to register with Scottish Social Services Council (SSSC) within 6 months of taking up employment in this role and to complete the required Post Registration Training and Learning (PRTL) over the first registration period. This post covers all Northeast Fife and hospitals in Perth and Dundee and therefore the ability to travel across Fife is essential. Before confirming to your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme. As this role requires SSSC registration, you will have three months to apply for registration. If appointed, you must start your application as soon as possible so you're registered within the required six months. If you need to achieve a qualification to maintain your SSSC membership, information will be provided if you are appointed.
