Participation in an out-of-hours on-call rota is required

Experience working with vulnerable adults and/or individuals with complex needs or challenging behavioursKey Responsibilities
Build positive and effective working relationships with external partners, including criminal justice services
Complete thorough needs and risk assessments to create tailored support plans
Provide advice, guidance and practical support to enable clients to access suitable accommodation and services
Ensure all new clients understand their rights, responsibilities, and the conditions of their licence agreement
