Overview
An opportunity has arisen for a Physiotherapist to join the Community Rehabilitation Team based in St Andrews Community Hospital. The role involves working with adults, mainly Older People, in the community setting to assess and treat in line with patients’ needs. Patients are seen in their own homes, in a homely setting or as an outpatient. The team focuses on community rehabilitation, preventing unnecessary admission, supporting earliest possible discharge from hospital and enabling patients to remain as independent as possible in their homes.
Responsibilities
* Provide physiotherapy assessment and treatment for adults, primarily Older People, in the community, home, or outpatient settings.
* Contribute to the goals of preventing hospital admissions, supporting early discharge, and maintaining patient independence at home.
* Work within the Community Rehabilitation Team to deliver evidence-based care and participate in service development as required.
* Supervise junior staff where applicable (Band 6 level).
* Be prepared to work at other community hospital sites as required.
* Travel throughout the area is essential due to the community-based nature of the post.
Qualifications and experience
* Degree in Physiotherapy and HCPC registration.
* Evidence of continuous professional development appropriate for the banding.
* Experience with audit and service development.
