Health & Wellbeing Practitioner - HMP Downview - SuttonPermanent£25,714.29Are you passionate about helping people overcome substance misuse and supporting mental wellbeing? The Forward Trust delivers trauma-informed, gender-responsive Health and Wellbeing services across Surrey prisons in partnership with NHS healthcare providers, and we’re looking for a dedicated Health & Wellbeing Practitioner to join our team at HMP Downview, a closed adult female prison.What you’ll do: Manage a caseload, providing tailored assessments, care plans, and recovery-oriented interventions. Deliver structured 1:1 sessions and group-work, supporting clients with substance misuse and low-level mental health needs. Provide harm reduction guidance and second signatory duties. Support release planning and transitional care for service users returning to the community. Work within a multidisciplinary team, collaborating with mental health, healthcare, and prison staff. Contribute to KPIs, audits, and the continuous improvement of our Health & Wellbeing services.What we’re looking for: Experience in substance misuse, mental health, or related health and social care fields. Strong interpersonal and communication skills, with the ability to work collaboratively. Flexibility to work on a rota including weekends. Ability to manage your caseload effectively and maintain accurate records.
