Working with system partners to create collaborative ways to improve physical health care services, activities and interventions in local communities, meeting the needs of patients where they call home. The division provides a broad portfolio across children’s, adults and specialist services, with a range of clinical and non-clinical roles in supportive teams focused on high quality care.
Our integrated teams of Nursing and Allied Health Professionals deliver care to people with significant physical health needs in settings including clinic-based, community inpatient, hospices, and in the patient’s own home and care homes, aiming to enable independence and reduce hospital admissions.
Services range from community nursing to public health and health promotion, including school nursing and health visiting, with specialist community services supporting care pathways such as Physiotherapy, Occupational Therapy, Speech and Language, Dietetics and Podiatry.

Responsibilities
* Contribute to delivering safe and effective care within community settings as part of a well-established, high-performing team.
* Support care pathways across services such as Physiotherapy, OT, Speech and Language, Dietetics and Podiatry as part of integrated community care.
* Participate in the efficient running of community health services and multidisciplinary teams.
Qualifications and Requirements
* Specialist Practitioner Qualified District Nurse (required for the post).
* A full UK driving licence and vehicle for business use (reasonable adjustments available in line with the Equality Act 2010).
* DBS check: costs paid by applicants; standard DBS £26.40 or enhanced £54.40, deducted from salary over first 2 months. Enrolment in the DBS Update Service is encouraged (£16 per year).
