We are seeking a compassionate andforward-thinking individual to join our Adult Community Service team as aSpecialist Doctor and clinically lead the expansion of our Hospital at Homeprogramme.

This pivotal role focuses on deliveringholistic, patient-centred care in the community, reducing hospital admissions,and enhancing integrated working across acute, primary, and social caresettings. You will collaborate with multidisciplinary teams to providehigh-quality medical care at home, ensuring patients receive timely support inthe most appropriate setting.

Key Responsibilities:

Lead the Hospitalat Home programme, supporting step-up and step-down care

Provide clinicalassessment, diagnosis, and treatment of acute and chronic conditions

Coordinate withED, inpatient wards, radiology, and primary care teams to ensure seamless care

Developpersonalised care and prevention plans, including Respect plans

Educate patientsand families to promote self-management and health awareness

Supervise and mentorjunior staff and advanced care practitioners

Contribute toquality improvement, clinical governance, and strategic planning

Job responsibilities

This post is offered on a part time (6 PA) basisalthough other less than full time applications will be considered. The postwill complement our existing consultant workforce, and individuals will workcross-site but the successful applicant will work predominantly supportingHospital at Home.

The Community Services Division is supportedby the Divisional Clinical Director, Allied Health Professionals, CommunityNursing and Therapists. The post being offered to complement the existingworkforce.

Clinical responsibilities will include:

Patient Care providing comprehensivemedical care to adults within the community, including diagnosis, treatment,and management of acute and chronic conditions.

Hospital at Home expansion clinicallyleading the Hospital at Home programme and its expansion developing effectivein reach services that facilitate admission alternatives, shortest possiblelength of stay and support patients in receiving care in the place they callhome through step up and step-down care.

Clinical assessment conduct in depthassessments and consultations, generating comprehensive management plans forthe current episode and future crisis / escalation.

Collaboration Work collaboratively withallied professionals, primary care networks, emergency department staff,inpatient and rehabilitation, divisions of medicine and acute care, radiologyteams (in hospital and CDCs), adult social care, VCSE and other stakeholders toensure coordinated and seamless care for patients. Promoting care at home whereappropriate.

Prevention and Care Plans Collaborate withpatients and multidisciplinary teams to develop comprehensive care plans,including Respect plans that honour patients' care preferences and values.Strong focus on preventive measures to enhance overall health outcomes andreduce the risk of hospital admissions.

Health Promotion Engage in community healthinitiatives and promote preventive care strategies that empower patients andtheir advocates to take charge of their health. Working to developneighbourhood models of care to ensure they are effectively integrated into theTrusts ways of working.

Patient Education Educate patients andfamilies about health conditions, treatment options, and lifestyle changes toempower them in their health journey. Supporting them to self-manage theirconditions to promote the most effective use of community resources.

Documentation Maintain accurate andup-to-date patient records, adhering to NHS standards and regulations.

Quality Improvement Participate in clinicalaudits and quality improvement initiatives to enhance service delivery andpatient care.

Supervision and Training Provide guidanceand support to junior staff and medical students, fostering a culture oflearning and development. Provide supervision and development support toAdvanced Care Practitioners and allied care professionals within the trust and(when agreed) educational support within community partners eg care homes.

Working arrangements will consist of thefollowing:

Hours across Monday to Friday currently 0830:1700.

Supporting Professional Activities in accordance with theTrust Framework, with at least aminimum of 1 SPAs included in the core 6PA timetable. Additional SPA or DCCtime may be commissioned in line with the skills of the successful candidateand changing requirements of the service.


Person Specification


Experience

* Shall have completed a minimum of 12 years medical work (either continuous period or in aggregate) since obtaining a primary medical qualification of which a minimum of six years should have been in a relevant specialty in the Specialty Doctor and/or closed SAS grades. Equivalent years experience in a relevant specialty from other medical grades including General Practice and from overseas will also be accepted.

* Awareness of legal responsibilities relevant to the role, such as around mental capacity and deprivation of liberty data protection, equality and diversity.
* Applies basic principles of public health; including population health, promoting health and wellbeing, nutrition, exercise, vaccination and illness prevention.
* Has personal insight into own strengths and weaknesses.
* Evidence of multidisciplinary team working

Professional Registration

* Please state full GMC number
* Full Registration and a Licence to Practice with the General Medical Council OR be eligible for the same and have this prior to commencement


Qualifications

* Medical Degree from recognised medical school
* MRCP or equivalent
* Prizes or Awards Achieved
* Audit or Case Presentations at a local, regional and national level
* Higher degree or qualifications such as a Masters
