This is a key role supporting the effective and timely discharge of patients and the smooth running of ward capacity.
You will play a central part in coordinating patient flow, acting as a single point of contact for bed availability and allocation within the ward. The role requires a proactive, organised and responsive approach, maintaining a real-time understanding of current and predicted capacity while ensuring discharge processes are well planned and delivered efficiently.
You will work closely with multidisciplinary teams both within the Trust and externally, helping to reduce delays and improve patient experience.
Priority will be given to colleagues identified as at risk through the Trust’s redeployment process.

Responsibilities
* Support the Ward Manager and Ward Coordinator in maintaining safe and effective patient flow
* Act as a central point of contact for ward bed availability and allocation
* Maintain accurate, real-time information on systems such as e-track and ward whiteboards
* Attend whiteboard rounds, identify required actions and actively manage discharge plans
* Focus on patients with predicted discharge dates within the next 72 hours to support timely discharge
* Liaise with Duty Managers when beds become available
* Coordinate admissions, internal transfers and external transfers
* Work with Transfer of Care teams and clinical specialties to minimise delays

Key responsibilities
* Assist in completing discharge checklists and ensure plans are clearly documented
* Chase clinical teams for discharge summaries and support timely provision of TTOs
* Ensure patients and carers are informed and prepared for discharge
* Expedite diagnostic requests and elevate delays appropriately
* Prepare documentation for patient transfers, including photocopying and organising notes
