This is an excellent opportunity for a qualified Adults Social Worker to join the Review Team based at County Hall, HU17 9BA. You will play a crucial role in reviewing care and support arrangements, ensuring adults receive appropriate, person-centred services that promote independence and wellbeing. What you will do Manage a varied caseload and complete statutory reviews to ensure care and support remain appropriate and effective. Undertake assessments and develop individualised care and support plans aligned with identified needs. Work collaboratively with health professionals, care providers, and partner agencies. Promote safeguarding by identifying risks and ensuring appropriate action is taken where required. Attend multi-agency meetings and engage with community resources to support holistic service delivery. Maintain accurate, timely, and compliant case records in line with statutory and organisational requirements. Requirements Qualified Social Worker with current Social Work England registration. Previous experience within Adult Social Care. Ability to manage reviews and assessments effectively within a statutory framework. Willingness to submit a CV for consideration and attend an interview. Photographic identification required on the first day for security compliance.
