An exciting opportunity has arisen for a band 6 cardiac rehabilitation nurse to join the trust-wide community cardiac rehabilitation team.
The post holder's base is to be negotiated; however, they will be expected to travel across Northumberland and North Tyneside to deliver care. A car driver is therefore essential.
Main duties of the job
The post holder will deliver cardiac rehabilitation to patients in their homes and leisure venues, involving lifestyle education and practical exercise sessions. A good understanding of cardiology, procedures, treatments, and their impact on quality of life is required to ensure the delivery of evidence-based, high-quality care. The successful applicant must demonstrate the ability to take on new challenges and actively participate in service developments. They should have the knowledge and skills to participate in audits, collaborate on research projects, and provide teaching and support to other staff members.

Job responsibilities
To work as a community cardiac rehabilitation nurse, providing a resource within Northumbria Healthcare NHS Foundation Trust, maintaining and improving communication pathways between primary, secondary, and tertiary care. To ensure a coordinated and seamless approach for cardiac patients and their families, linking with partnership organisations. To manage patients with a wide range of cardiac conditions, assessing, planning, implementing, and evaluating care.
Person Specification
Qualifications
* Registered Nurse (Adult Branch) Level 1 NMC Professional Register
* Recognised degree (or working towards)
* Teaching certificate (or willing to work towards)
* Mentorship qualification or relevant equivalent (essential; expected to be completed within 18 months)
* BACPR health professional course or willingness to undertake
