Main duties of the job
The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients. We are currently a GP partnership of 30 practices with 38 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton and Shropshire.

* Key Roles and Functions
* To work autonomously, providing a high quality, safe and supportive environment for patients within the scope of individual accountable practice.
* Work within Practice guidelines, policies, protocols and procedures, reporting outcomes to appropriate clinical staff; adhere to infection control, equality and diversity, and health and safety agendas.
* Maintain patient records to the agreed Practice standard and in accordance with professional record keeping standards (NMC 2009).
* Administer medicines in accordance with a non-medical prescribing framework, patient specific direction or patient group direction in line with medicines management standards (NMC 2007).
* Organise, manage and prioritise own workload to ensure patient care is paramount.
* Develop the role using evidence-based practice and continuously improve own knowledge as defined in PREP and revalidation guidelines.
* Job Responsibilities
* Diagnose and treat patients presenting with minor illness; triage and refer as needed to other members of the primary healthcare team.
* Advise patients on general healthcare and minor ailments; refer to other team members as necessary.
* Prescribe medicines in accordance with NMC Standards for Medicines Management and related guidance.
* Ensure clinical practice is safe and within the boundaries of competence.
* Providing Assessment, Screening and Health Education
* Minor Illness/Minor Injury clinics; chronic disease care planning; frailty and care homes responsibilities; palliative/end-of-life care and DNACPR/Respect forms as required; structured medication reviews; health screening and blood tests as needed.

* Willingness to undertake additional courses and training; attend meetings, with some study outside working hours.
* Undertake regular clinical supervision with a GP; adapt working practices to service development; flexibility across sites as required; performance appraisals annually.
Person Specification
Essential Qualifications
* Professional Registration
* Masters degree required for qualification post December 2020 (RCN Credentialing for Advanced Level of Nursing Practice)
* Postgraduate diploma or degree for Advanced Practice Qualification up to December 2020
* Independent Prescriber
Desirable Qualifications
* Teaching & Mentoring qualification
* Advanced Life Support qualifications
Experience
* Essential: minimum 5 years post-registration experience at senior level (Band 7 or above) in a specialty area; evidence of ongoing CPD
* Desirable: evidence of teaching in a variety of environments
Knowledge & Skills
* Essential: Advanced Clinical Assessment skills; evidence of research & audit
* Desirable: publications and dissemination of knowledge in wider healthcare organisations
