As a Paediatric diabetic nurse, the post holder will work in collaboration with the extended team to ensure care delivery to CYP with Diabetes. The post holder will work in line with national guidance to understand current service provision for children with Diabetes; taking into account the BPT National service framework, government, regional and Trust initiatives. The post holder will work as part of the team to ensure a timely, clear transition to adult services and develop professional partnerships with adult and paediatric services to ensure the implementation of effective and efficient transition pathways for patients. As a Paediatric Nurse, successful candidates should be prepared to support hospital staff, attend clinics and oversee care of in-patients and whilst acting as a specialist nurse and with the support of the B7 develop and lead in the delivery of care. The Post holder will participate in training for the wider trust as needed.
* Provide professional expert clinical care and expertise within diabetes services
* Be a key contributor to the strategic development of the diabetes service taking account of the BPT National Service Framework, government, regional and Trust initiatives. This also includes Peer Review agreements with the ICBs.
* Work in collaboration with medical and other multi-disciplinary staff to provide a comprehensive patient focused service.
* Ensure nurse clinics are provided consistently and efficiently.
* Initiate and participate in the teaching programmes of the diabetes and team and ward teams.
* To advise on the management of and facilitate early discharge for patients admitted with diabetes, either as newly diagnosed or as part of their ongoing condition. Providing educational support as required.
* Be the focal point for inpatient referrals to the diabetic team, providing first line advice to doctors from other teams.
