Job Description

Job Title: Aging Adult Case Manager
Location: Upper West Side
Schedule: Monday - Friday, 8:00 AM - 4:00 PM (35 hours/week)
Occasional extended hours one evening per month

Overview:
We are seeking a dedicated and compassionate Aging Adult Case Manager to support the well-being and independence of older adults in our community. This role is vital in providing personalized case management and holistic support services to help seniors age safely, comfortably, and with dignity in their own homes. Key Responsibilities:

* Provide direct support and advocacy for older adults across health, mental health, food access, and housing needs.

* Manage a high caseload and conduct regular assessments and home visits, particularly for homebound seniors.

* Deliver individual counseling and make appropriate referrals to community and healthcare resources.

* Monitor clients for cognitive changes, coordinate interventions, and liaise with medical professionals.

* Identify and report potential elder abuse or neglect.

* Accompany clients to medical and other essential appointments, advocating on their behalf.

* Maintain accurate, timely documentation in the DFTA online database.

* Facilitate support groups tailored to the emotional and practical needs of seniors.

* Support family members and caregivers by providing resources and referrals.

* Collaborate with onsite nurses to coordinate care and ensure medication management.

Qualifications:

* Bachelor's degree (BA, BS, or BSW) in Social Work, Psychology, Gerontology, or related field.

* Bilingual proficiency in Spanish, Cantonese, Mandarin, or Russian required.

* Prior experience working with older adults.

Preferred Qualifications:

* Experience working in an adult or senior center setting.
