Position Overview
The Advanced Nurse Practitioner will work as an integral member of the clinical team, delivering advanced, evidence-based assessment, diagnosis, and treatment for patients presenting with a broad spectrum of undifferentiated, acute, and chronic conditions. The role requires a high level of clinical expertise, strong decision-making skills, and the ability to work independently while also contributing effectively within a team setting.
The ANP will be responsible for managing their own clinical workload, including urgent and same-day presentations, and will play a vital role in ensuring timely access to care for patients. A strong emphasis is placed on patient-centred care, health promotion, and preventative medicine.
Key Responsibilities
Deliver comprehensive clinical assessments, diagnoses, and treatment plans for patients presenting with undifferentiated and complex conditions
Independently prescribe medications in line with practice formulary, national guidelines, and best clinical practice
Manage a range of clinical presentations, including urgent/on-the-day appointments, minor illness, chronic disease reviews, and long-term condition management
Request, interpret, and take appropriate action on diagnostic investigations, including pathology results and imaging
Provide health promotion, screening, and patient education to support preventative care and self-management
Work collaboratively with GPs, practice nurses, healthcare assistants, pharmacists, and allied health professionals to ensure holistic patient care
Maintain accurate, clear, and contemporaneous clinical records in line with professional standards and regulatory requirements
Contribute to clinical governance, quality improvement initiatives, and the ongoing development of practice services
