Associate Specialist Palliative Care Nurse
We are seeking a dedicated professional to provide evidence‑based palliative care, advice, and support to patients diagnosed with cancer or other life‑limiting illnesses, along with their caregivers. In this role you will be accountable for your own professional actions without direct supervision.
Responsibilities include:
* Plan and deliver education to residential and nursing homes to maximise care for palliative patients.
* Deliver high‑quality palliative care as part of a seven‑day service, collaborating with nursing, medical and allied health professionals.
* Provide guidance on clinical and ethical issues, promoting palliative care education across disciplines.
* Support clinical audits and contribute to clinical governance.
* Work with registered nurses in care homes to educate in palliative and end‑of‑life care.
* Manage a caseload of patients and caregivers, assessing and implementing individualized care programs.
* Provide expert palliative care advice and support to patients, caregivers and healthcare professionals, utilizing specialist knowledge of pain and symptom management, psychosocial needs and ethical dilemmas.
* Assist patients in achieving self‑care when appropriate and educate caregivers to provide effective support at home.
* Establish and maintain effective communication with patients, caregivers and professionals, developing therapeutic relationships.
* Act as an advocate for patients, families and caregivers when necessary, ensuring that their needs and wishes are prioritised.
* Work flexibly and during unsocial hours to meet the demands of the service.
