As our Lead Support Worker, known internally to us as Support Coordinator, you\'ll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community. Responsibilities
* Supporting all, and holding a caseload of customers with a variety of support needs but primarily homeless prevention. You can expect to see secondary needs of substance misuse, mental health and physical health.
* Creating support plans with your customers and coordinating our small team of Support Workers working to the plans
* Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
* Carrying out risk assessments, support planning, goal setting and regular reviews.
* Cuppas, chats and catch ups. You will also be offered toast by our resident Toast specialist, Sam. There\'s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.
* Passion to support our customers to live their best life, working collaboratively with an eye for detail.
* Experience of creating person-centred support plans and supporting colleagues in working to the plans.
* Experience of coordinating and assessing customer referrals.
* The ability to work on your own initiative, remain calm under pressure and have a resilient approach
* To get from A to B, you\'ll need a vehicle insured for business purposes. The great news is that we\'ll pay your mileage!
* Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
* You\'ll need an Enhanced DBS check done and we pay for that.

Shifts and requirements
Early shifts are 09.00-17.00; middle shifts tend to be 11.00-19.00; and lates which are 13.00-21.00. Although you\'ll work shift, we do get together regularly for training and team meetings.
