Job Summary
Patient Service Coordinator – a non‑clinical role in the Community Heart Failure Service that supports patient care and multidisciplinary teams. The role is part‑time, 30 hours per week, with a pay rate based on the Real Living Wage or a supplement if below national A4C rates.
Responsibilities
* Manage timely and appropriate patient bookings, ensuring patients are seen at the right time by the right clinician.
* Coordinate follow‑ups, obtain diagnostic reports and handle referrals, improving continuity of care and supporting the clinical team to prevent readmissions.
* Enhance access and continuity of care for patients with heart failure.
Qualifications & Experience
* Essential
o Good general education.
o Sound literacy and numeracy skills.
o Formal secretarial/administrative training.
o Word processing training in Microsoft Word for Windows.
o NVQ Admin Level 3 in Administration or equivalent experience.
o Secretarial and administrative experience in an office environment, including remote working.
o Advanced keyboard skills; experience of taking minutes, data entry and arranging meetings.
o Experience with Microsoft Office and associated documentation.
o Ability to manage patient distress in person and on the phone.
* Desirable
o European Computer Driving Licence.
o Knowledge of NHS structures and processes.
o Prior experience working in a healthcare setting.
o Wide experience of various database systems and design.
Knowledge & Expertise
* Understanding of the need for confidentiality at all times.
* Understanding of Equal Opportunities.
* Full understanding of how to create and structure office systems, e.g. Emis Caseload stationery ordering.
* Understanding of PC file/document management principles.
* Awareness of the high levels of distress service users may present with.
Additional Information
Salary: £24,937 to £26,598 per annum (pro‑rata). Contract: Permanent, part‑time 30 hours per week. Start date: Fixed start date each month with a 3‑week P2C induction programme.
