Job overview

We are delighted to welcome applications for the role of trainee Advanced Clinical Practitioner within the community ACP team (East) based at Lightwood house.

The successful candidate will be supported to undertake a fully funded MSc in Advanced Clinical Practice and role specific training in order to develop capability as an independent, autonomous, accountable practitioner. They will develop skills to enable them to work on their own initiative and take a lead role in clinical decision making, working collaboratively with the wider multi-professional team.

Once qualified they will be supported to progress through the local CASP gateway where the role then attracts AfC band 8a. The qualified ACP fulfils a senior role in the clinical team with the autonomy to initiate action based on their interpretation of policies/guidelines, seeking advice if required.

Main duties of the job

The community ACP role offers variety through working across Hospital@Home (Virtual Ward), Urgent Community Response and complex case management services.

The role will be structured around the four core functions of advanced practice:

1. Expert advanced level practice (with a minimum clinical focus of 80%)
2. Professional leadership and consultancy
3. Education, training and development of self and others
4. Practice and service development, research, and evaluation

The trainee ACP will be supported with off the the job learning and support for their academic studies. When qualified our ACPs should have a minimum of 12hrs per month (pro rata) for work relating to the non-clinical pillars and to ensure maintenance of competency in all areas of the role. This will facilitate improvements to patient care through the education of others, and quality/service improvement associated with leadership and research activities.

The successful candidate would be expected to be flexible and provide cover where needed city wide / dependant on service provision, covering hours between 8am – 8pm over a 7 day period.

The Combined Community and Acute (CCA) care group is the largest of 12 care groups in the Trust. The Care Group has a workforce of over 2,500 people, and a budget of approximately £120 million. There are 39 discrete services, with over 85 teams, and we have 363 beds (128 in the community) plus virtual wards.

We work in around 38 venues including 5 hospital sites, 16 staff vases in community venues, 4 bedded units in community and multiple venues in the community for delivery of care to patients.

Our community ACP team sits within the Integrated Community Care (ICC) Directorate. ICC is the largest Directorate within CCA and supports people in their own homes and communities by providing high quality health services promoting independence and improving quality of life. The services are delivered by multidisciplinary teams and include primary care out of hours service, integrated community teams (nursing and therapy), podiatry services, Single Point of Access (SPA), admissions avoidance and discharge to assess services.

Education and Qualifications

Essential criteria

5. Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience
6. Relevant Masters’ degree in Advanced Practice or NHSE Centre for Advancing Practice Advanced Practice Digital Badge (Band 8a)
7. Willing and able to successfully complete a full Masters in Advanced Clinical Practice - which may be via the apprenticeship route (Band 6 & 7)
8. Advanced life support qualification (ALS/ILS) or willingness to undertake (Band 6 & 7)
9. Must be able to provide written evidence of GCSE English and Maths at C or above (or equivalence) at the time of interview (apprenticeship requirement) (Band 6)
10. Honours degree recognised in the UK at 2.2 or above in a related health care subject, or an equivalent qualification (HEI requirement) (Band 6)
11. Advanced Physical Assessment module, Non-Medical Prescribing (if part of proposed role) (Band 7 & 8a)
12. Advanced life support qualification (ALS) or Intermediate life support qualification (ILS) dependant on role (Band 8a)
13. Advanced life support qualification (ALS/ILS) or willingness to undertake and successfully complete (Band 6 & 7)
14. Post Graduate qualification in Advanced Practice (Band 7 & 8a)
15. Evidence of relevant continuous professional development

Desirable criteria

16. Recognised Teaching & Assessment Qualification or equivalent experience
17. Recognised Leadership Award/qualification or equivalent experience

20. Experience of delivering training / education to others
21. Significant post registration experience (HEI requirement) with Relevant setting (ie community / primary care / geriatric / frailty)
22. Significant relevant experience as qualified ACP (Band 8a)
23. Must be able to evidence and demonstrate experience of ability to act as an independent, senior decision maker in a clinical team using published specialist curricula where available (Band 8a)
24. Evidence of portfolio of practice and sign off of competence of knowledge, skills and experience across all 4 pillars of advanced practice using CASP (or equivalent if external applicant) (Band 8a)
25. Experience of undertaking service improvement activity (Band 8a)
26. Evidence of contribution to audit and research activity (Band 8a)
27. Experience of training, supervising and assessing competence of junior colleagues (medical staff / ACP's / NMAHPs) (Band 8a)
28. Evidence of multi-disciplinary team working

Desirable criteria

29. Experience of undertaking service improvement activity (Band 6 & 7)
30. Evidence of contribution to audit and research programs
31. Experience of training, supervising and assessing competence of junior colleagues (medical staff / ACP's / NMAHPs) (Band 7)

Skills and Knowledge

Essential criteria

33. Good base knowledge of pathophysiology of common conditions seen in the speciality and their management
34. Able to analyse and synthesise patient information to formulate appropriate diagnoses and management plans
35. High standard of written and verbal communication - able to present and receive complex information and communicate effectively with all grades of staff and patients / families
36. Can prioritise and manage own workload and support junior personnel in managing theirs when faced with conflicting pressures
37. Ability to manage self and maintain professional conduct in difficult and challenging situations

Desirable criteria

38. Effective leadership, negotiation and reasoning skills with the ability to influence, coach and motivate others
39. Knowledge and experience of appraisal processes
