The NHS is seeking a Specialist Frailty & PCN Pharmacist to provide medicines optimisation across Kent community hospitals and GP practices. This is a full-time permanent role that involves a patient-centered approach to ensure that patients take the right medicines while minimizing hospital admissions.
Successful candidates will hold a Master of Pharmacy degree, be registered with the General Pharmaceutical Council, and have substantial pharmacy experience in community or hospital settings. Competitive salary between £49,387 to £56,515 per annum, pro-rata.
