Inline with the NHS Long Term Plan and Ageing Well agenda, the role focuses onpreventative and anticipatory care. The nurse will work closely with GPs,pharmacists, social prescribers, community services, and voluntary sectororganisations to deliver integrated care that promotes independence, dignity,and quality of life for older people in their own homes.


Main duties of the job

The Frailty Nurse is a pivotal member of the Primary Care Network (PCN) team, responsible for delivering comprehensive, proactive, and personalised care to patients living with moderate to severe frailty, including those who are housebound. Working autonomously and collaboratively, the post holder conducts holistic assessments in patients homes, co-produces care plans, supports long-term condition management, and works to reduce unplanned hospital admissions.

The PCN comprises of 4 Practices across the Forest of Dean. ForestGreen PCN consists of Dockham Surgery, Drybrook Surgery, Forest Health Care andMitcheldean Surgery. The practices have a combined geographic area made up ofaround 29,000 patients. Our PCN iscommitted to developing a skilled multidisciplinary workforce that can maximisethe delivery of safe, high quality and value for money healthcare services forthe benefit of our local population. Youwill be joining an enthusiastic team of clinicians and administrators. You willbe well supported with ongoing professional development (training support, CPD,and peer support). The role will contribute to improving the quality of care ofour patients across the network of practices.

Job responsibilities

Person Specification


Experience

* Experience of working in a primary care environment
* Experience of chronic disease management
* Experience of working as a practice nurse or community nurse


Qualifications

* Degree in nursing and registered with the Nursing and Midwifery Council (NMC)
* Post graduate diploma or degree (primary care)
* Independent prescriber


Clinical and Other Skills

* Wound care/removal of sutures and staples
* ECGs
* Venepuncture
* Chaperone procedure
* Requesting pathology tests, processing the results, advising patients accordingly
* Diabetes
* Asthma
* CHD
* Immunisations (routine, childhood and travel)
* Frailty Index- Rockwood trained
* Understand the importance of evidence-based practice
* Broad knowledge of clinical governance
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understand when to refer to GPs
* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena
* Knowledge of health promotion strategies
* Understand the requirement for PGDs and associated policy
* Excellent communication skills (written and oral)
* Clear, polite telephone manner
* Competent in the use of Office and Outlook
* Effective time management (planning and organising)
* Ability to work as a team member and autonomously
* Good interpersonal skills
* Problem solving and analytical skills
* Ability to follow clinical policy and procedure
* Experience with audits and able to lead audit programmes

* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena
* 6-CIT trained
