We are currently recruiting for an experienced and motivated Social Worker to join Warrington’s First Response Team on a 3-month contract. This is a fantastic opportunity to work within a fast-paced front-door service where you can make a real difference by ensuring adults receive the right support at the right time.
The First Response Team plays a vital role in preventing, reducing and delaying the need for long-term care, supporting individuals at the earliest stage of their contact with Adult Social Care. You will be part of a supportive team responsible for triaging new referrals, safeguarding concerns, and coordinating timely assessments to promote independence and wellbeing.
The Role As a Social Worker in the First Response Team, you will manage a short-term caseload, focusing on screening referrals and carrying out key statutory assessments. You will work closely with individuals, families, carers and partner agencies to ensure people receive the most appropriate support and intervention.
Your responsibilities will include:
Screening and triaging new Adult Social Care referrals
Completing Care Act 2014 assessments to determine eligibility
Undertaking Carers Assessments, Mental Capacity Assessments, and Best Interest decisions
Screening and prioritising Adult Safeguarding concerns, including Section 42 enquiries
Supporting Continuing Healthcare (CHC) processes
Working within the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983
Managing short-term interventions and reviewing care arrangements
Applying a strengths-based approach to promote independence and wellbeing
Working collaboratively with partner agencies to deliver positive outcomesAbout You We are looking for a confident and resilient Social Worker with experience working with adults with a range of complex needs. You will thrive in a busy duty environment and be confident applying legislation in practice.
You will have:
Experience in Adult Social Care
Strong knowledge of Safeguarding and risk management
Experience completing statutory assessments
The ability to work effectively within a fast-paced team environment
