Overview
Social Worker | Full Time | Permanent | Urgent Care and Short Term Support | Fylde & Wyre. 37 hours per week. Salary £36,363 - £40,778 per annum. This role is part of Lancashire County Councils Short-Term Support Service, working to support people to live independently in their homes.
Responsibilities
* Provide holistic, face-to-face Care Act Assessments, reviews and support planning using a strengths-based approach.
* Work within the team as a single point of access to community intermediate care services, including reablement and residential rehab, 7 days per week.
* Support hospital discharges and collaborate with the wider multi-disciplinary team to prevent hospital admissions and enable people to remain at home.
* Utilise the Discharge to Assess model to ensure assessments are conducted away from the hospital environment.
* Contribute to achieving Living Better Lives in Lancashire by delivering innovative practice and supporting people to live as they choose.
Requirements
* Social Work England (SWE) registration to be appointed as a Social Worker or have applied to register with SWE.
* Car required for business use; an insurance policy that covers business use. We may consider applicants who cannot drive due to a disability.
* Willingness to undertake a Disclosure and Barring Service (DBS) check if appointed, and to provide information as part of the recruitment process.
* Commitment to safeguarding and to working within the Rehabilitation of Offenders Act framework as applicable.

* Qualified Social Worker with registration or eligibility to register with Social Work England (SWE).
