Job title
Deputy Sister / Senior Nurse
Responsibilities
* Deliver high-quality holistic care to prevent unnecessary hospital admissions, alongside specialist nursing interventions
* Support timely discharges and promote recovery at home
* Coordinate and provide day-to-day support within the Community Response Team, ensuring effective communication and collaboration with registered and unregistered staff
* Engage patients and families using health coaching techniques to set realistic, empowering goals
* Co-ordinate activities in the CRT team and contribute to assessment of patient care needs, development, implementation, and evaluation of programmes of care

Job responsibilities
* The post holder will co-ordinate activities in the CRT team ensuring effective communication is always maintained, providing support to registered and unregistered staff within the ward/department.
* This post will play a key role in the assessment of patient care needs and the development, implementation, and evaluation of programmes of care.
* The post holder will act as a role model, promoting quality evidence-based practice, developing the nursing team, and ensuring effective management of the ward/department in the absence of the Team Leader.
* The post holder will work in partnership with the Team Leader and the team to ensure that high standards of nursing care are always provided.
* The post holder will provide day-to-day support to the non-registered clinical support team, including Nurse Associates and Generic Support Workers within the York Community Response Team.

Education, Qualifications and Training
* Registered Nurse
* Recognised teaching / assessing qualification
* Evidence of continuing professional development and implementation in clinical practice in the community within the last 12 months
Experience and Knowledge Required
* At least 3 years experience at Band 5 or equivalent
* Insight into clinical speciality - community
* Insight into rehabilitation and re-ablement issues
* Insight into current issues in nursing / professional issues
Skills and Attributes
* Excellent organisational and prioritising skills
* Ability to communicate effectively with patients and the multidisciplinary team, handling difficult, complex situations calmly and promptly
* Reliable, adaptable to change and able to work on own initiative or as part of a team
* Good verbal and written communication skills including a professional telephone manner
* Ability to teach, supervise and assess registered / unregistered staff
