We have an exciting opportunity for a Primary Care Nurse- Cardiac Lead to join our Primary Care team at HMP Wakefield. HMP Wakefield is a category A mens Prison located in Wakefield, West Yorkshire.
Hours
The site currently offers a 24 hour service and have shifts patterns across these hours
The role will require working 4 out of 7 days on a rota basis. This includes evenings, weekends and possibly some night shifts
Salary
This position is full time, youll receive an annual salary up to 46,200per annum - depending on experience.
Qualification
1. Registered nurse Adult with current NMC registration
2. Qualifications in cardiac rehab and the heart manual would be beneficial

Main duties of the job
As a Primary Care Nurse- Cardiac Lead, your responsibilities will vary. You will
3. Be responsible for the monitoring all cardiac long term conditions, cardiac rehab, heart manual and the management of the emergency bags and equipment.
4. Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
5. Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
6. Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team

Qualifications required
