Senior Support Time, and Recovery Worker
Detailed job description and main responsibilities:
* MDT and inter-agency working: To work as part of the multi-professional team supporting the planning, implementation, and evaluation of patient care needs in collaboration with patients, carers, families, and other community/inpatient services.
* Personalised care planning: To support within the MDT, working closely with patients and their carers; assisting patients in recovery goals, including practical/daily living tasks and social inclusion activities; ensuring care plans are updated as needs change.
* Partnership and communication: To maintain effective links with other mental health services and community agencies, ensuring continuity of quality clinical care. To support patients in accessing local community resources and developing knowledge of these resources.
* Referral support: To assist patients in self-referring or completing referrals to community services such as Age UK, London Fire Brigade, and social clubs, and support initial engagement if required.
* Patient engagement: To escort patients to appointments if needed, participate in team meetings and reviews, and contribute to care planning. To support multidisciplinary team colleagues and share information and skills appropriately.
* Community support and discharge: To work with home carers and social services to promote recovery and support timely discharge by identifying and facilitating access to community support services.
Clinical skills: Demonstrate risk awareness and support risk assessment and management. Support carers and families regarding their rights and involvement. Identify patient interests and key relationships to aid recovery. Maintain clinical records according to standards, ensure confidentiality, and keep mental health resources up to date. Report incidents and support relapse management.
Record keeping and communication: Maintain accurate electronic records, ensure confidentiality, and communicate changes to the team. Work within a recovery framework to help patients achieve their recovery goals.
Personal development: Reflect on practice through supervision and appraisal, support team well-being, undertake mandatory training, and contribute to service development, audits, and research.
Personal skills: Work with distressing issues compassionately, develop positive relationships, promote a person-centred approach, and manage workload effectively.
