PCN Clinical Pharmacist (Maternity Cover)
Job Summary
An exciting opportunity has arisen for a forward‑thinking and motivated Clinical Pharmacist to join our Synergy PCN in Uxbridge on a 12‑month fixed‑term contract for maternity cover. Please note that this role requires full on‑site working for five days a week.
The successful candidate will work across a number of practices developing this role closely supported by the GPs. They will be part of a group of Clinical Pharmacists under the guidance and supervision of a delegated clinical supervisor.

Salary
£46,000 to £59,000 a year (dependent on experience)
Key Responsibilities
* Develop and run processes for repeat prescription authorisation, management of medicines on transfer of care and systems for safer prescribing.
* Work directly with patients to assess and treat conditions, manage long‑term conditions and promote self‑care.
* Undertake clinical medication reviews with patients within the practice, care homes and/or domiciliary visits to proactively manage polypharmacy.
* Provide patient‑facing long‑term condition clinics (e.g., respiratory, cardiovascular, diabetes). Offer clinical leadership on medicines optimisation and quality improvement.
* Support medication optimisation in care homes and domiciliary settings, working with staff to improve safety of medicines ordering and administration.
* Produce and implement repeat prescribing policies, manage repeat prescribing re‑authorisation process and ensure appropriate monitoring tests.
* Identify and support patients at high risk of harm from medicines; audit prescribing practices and implement changes in line with NICE and MHRA guidance.
* Provide education and training to the primary healthcare team on therapeutics and medicines optimisation.
* Collaborate with the Primary Care Network Multidisciplinary Team, CCG colleagues, community pharmacists and other stakeholders.
* Work with line manager on personal and professional development, clinical supervision and education programmes.
Minimum Requirements
* Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council (GPhC).
* Minimum of 2 years experience as a pharmacist.
* Experience and awareness of common acute and long‑term conditions likely seen in general practice.
* Working towards or holding an independent prescribing qualification (not mandatory).
* Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
Desirable Qualifications
* Masters degree in Pharmacy (MPharm) or equivalent.
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
* Independent prescriber or working towards/intent of gaining an independent prescribing qualification.
