Mendlesham Medical Group has a fantastic opportunity for an experienced or a newly qualified practice nurse to join our team on a part-time basis, for 18 hours per week on Thursdays and Fridays. The main responsibilities of the role include: Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, i n line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations. Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review and management. Running asthma, COPD and diabetes clinics and reviews. About the Candidate We welcome applications from both experienced and newly qualified practice nurses who share our commitment to excellent patient care, teamwork, and innovation. All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN. Ideally having completed the CNO002 Discovering a Nursing Career in General Practice module or the Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) however we would still be interested in those applicants with equivalent experience or qualifications. Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, dressing and wound care, and recording spirometry. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS.
