Overview

As a Dementia Adviser, you will have the rewarding opportunity to provide support, information, and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs. In this community and home based role, you will offer support to your clients by phone, letter, email and in person home visits. You will also assist service users to access other services, providing signposting and referrals. You will work closely with the NHS Memory Assessment Service in clinics across East Lancashire and Blackburn with Darwen to offer support to people following a dementia diagnosis. You will help organise and attend dementia support hubs and other networking events.


What you’ll focus on

* Supporting people affected by dementia, families and carers to access vital services.
* Attending diagnostic clinics run by the Memory Assessment Service.
* Improving people’s sense of well-being, enabling them to have more control over their lives.
* Assisting with the identification of needs, providing information and access to relevant services.
* Organising and attending dementia support hubs and other networking events.
* Promoting our amazing service and building relationships with a range of health and social care professionals.

Interviews will take place on 25th & 26th September 2025.
