Job Description
We are seeking a skilled and detail-oriented Care Partner to join our Case Management team based in the Aspley Office.
The Care Partner will be responsible in supporting customers in the delivery of service that meet the Aged Care Standards.
This is a full-time role, Monday to Friday, with hours between 8:00am and 4:00pm.
Duties Of The Role Include
* Work within a close and dynamic team to coordinate the delivery of our high standard of care to clients. Our intention is to meet and exceed client expectations and meet business requirements.
* Onboard new Support at home clients, as well as conducting regular care plan reviews in accordance with the clients changing needs.
* Communicate with staff and stakeholders regarding enquiries, referrals and follow ups via phone and email.
* Relationship building with clients and their families.
* Monitor Clients budgets.
* Enable our clients to lead fulfilling, independent lives through value focused relationships and quality support services.
* Partnering with Stakeholders (Associated providers) to ensure services are delivered smoothly and meet client needs and expectations.
* Maintaining accurate client records by documenting interactions, updating information promptly, and sharing relevant details with the team to ensure continuity of care.
* Providing Caregivers / nominated support persons with timely feedback, essential updates and ongoing encouragement to support exceptional client care.

Desired Skills And Experience
* Tertiary qualifications in a related discipline in Health, Community, Disability or Business Administration (for example Certificate III Individual Support or Nursing)
* Strong knowledge of Aged Care Quality Standards
* Willingness to learn and / or previous experience in Care management of government funded packages, community resources, and relevant legislation in the provision of aged care services
* Current Police Check (within the last 6 months)
* Current First Aid / CPR
* Valid QLD drivers’ license
* Full Work Rights
* NDIS Worker Screening
