Overview
The post holder is responsible for the implementation and monitoring of Continuing NHS Health Care funding (CHC) in Care Homes/domiciliary settings and NHS Funded Registered Nursing Care (FNC) in Care Homes (Nursing) in accordance with National Guidance. The post holder will co-ordinate, scrutinise CHC applications and participate in CHC and FNC reviews and attend and participate in Multi Disciplinary Team (MDT) meetings within the area/department where required. The post holder will manage the local resolution process within the area/Department (i.e. meet and liaise with families to discuss panel outcomes when individuals challenge eligibility decisions) and will oversee Inter Agency Disputes (meeting and liaising with the appropriate Local Authority Team Manager where there is a dispute regarding eligibility for CHC funding). The role involves collaboration with statutory agencies where safeguarding issues are identified and attendance at POVA/Strategy Meetings. It also entails working with Discharge Liaison Nursing Teams / Community Hospitals / District Nurses to ensure safe and timely discharge/transfer of patients.
Main duties of the job
* Work across professional and organisational boundaries, in particular with Independent Care Homes, Social Services staff and other Health Boards and Primary Care Trusts when patients are moving in/out of the area.
* Develop and maintain sound relationships within Statutory/Voluntary/Independent sectors including health care professionals, GPs and Local Authority areas across BCUHB.
* Promote excellent customer care throughout the Continuing Health Care process, ensuring local resolution of concerns where possible, including communication of highly complex, sensitive information to external organisations and the public.
* Negotiate with clients/families and members of the public, delivering complex information in highly sensitive and emotional situations (e.g., CHC applications/reviews with potential financial implications for self-funding clients).
* Maintain accurate case records and ensure panel decisions are communicated clearly and sensitively within agreed timescales as per The National Framework for Implementation in Wales.

* Degree or equivalent level of experience in health.

* Essential: Significant post registration experience; evidence of leading and developing service improvement and experience of improvement measures; specialist knowledge in relevant field.
