The PCH is a multidisciplinary team delivering healthcare to people in South Waveney, including the towns of Beccles, Bungay, Halesworth, Southwold, and Kessingland. The team’s combined skills enable them to respond to the changing needs of patients and support those who might otherwise be admitted to hospital to remain at home. You will become an active member of this team of clinicians. We are happy to provide preceptorship for a newly qualified Registered Nurse.
The PCH team provides community services 7 days per week, from 07:00 to 20:30.
The core shift is 08:30-16:30, with other shifts included in the rota: 07:00-15:00 and 12:00-20:00. Weekend and bank holiday work is on a rotational basis. The post is offered at 37.5 hours per week, worked in 7.5-hour shifts over 5 days. We support flexible working arrangements, and other shift patterns can be considered.
Main duties of the job
As a community nurse, you will provide excellent, person-centered care to adults in their own homes, residential care, or clinics. This includes a mix of nursing and reablement interventions to support patients in maintaining their health or recovering from illness. You will be supported by senior nurses to develop the clinical skills necessary for community care and will eventually manage your own caseload.
You will complete holistic assessments for referred patients and participate in delivering nursing interventions, which may include:
* Holistic assessments including environmental, moving and handling, pressure area risk, and nutritional needs
* Urinary catheterization
* Administration of medication via injection
* Wound care and specialist leg ulcer management
* Palliation and end-of-life care
* Supporting patient self-care using health coaching techniques

South Waveney Primary Care Home is a multidisciplinary team of health and social care professionals working with primary care, social services, care providers, and voluntary organizations to help patients remain in their homes whenever possible.
