Job overview

Job Title: Advanced Clinical Practitioner

Band: 8a

Location: Frailty Assessment Unit (FAU) at County Hospital

Hours of Duty: 37.5 hours per week (hours worked over a month, includes long shifts, weekends and bank holidays)

FAU is a bespoke 6 bedded frailty assessment unit and you will work in an MDT setting. The MDT comprises of consultants, ACPs, nursing staff, dedicated therapists and IDH support.


Main duties of the job

The Advanced Clinical Practitioner Team in FAU is at the forefront of Same Day Emergency Care (SDEC); with this in mind, an opportunity has arisen for a motivated Advanced Clinical Practitioner to become part of a highly skilled established team.

The successful candidate will be based in the Frailty Assessment Unit at County Hospital with close integration into Emergency Medicine and front-door services.

You will support admission avoidance and holistic patient management for patients presenting with frailty syndromes enabling safe, person-centred and efficient care pathways from the Emergency Department (ED) into either early supported discharge or further inpatient care.

Person specification


Education and Qualifications

* Current registration with professional body such as NMC or HCPC
* MSc degree in advanced clinical practice or equivalent MSc
* Registered independent non-medical prescriber
* ALS provider or intermediate life support provider (dependent on area of practice)
* Mentor/assessor or formal teaching qualification
* Accreditation with the Centre for Advancing Practice


Knowledge, Skills and Experience

* Currently working as a qualified ACP
* Evidence of continued professional development in all four pillars of advanced practice (clinical, leadership & management, education and research)
* Formal leadership and management experience
* Excellent written and communication skills
* Evidence of delivering training programmes and presentations to various levels of staff


Personal Qualities

* Positive mindset in respect of organisational change; learning and continuous improvement
* Acts to support and enable effective teamwork
* Effective interpersonal and intrapersonal skills including high degree of self-awareness and self-regulation
