Position:

An exciting opportunity has arisen for a Senior Pharmacist - Intermediate Care based at The Johnnie Walker Bond on a 12 month fixed term / secondment to cover maternity leave.

What you will do:

The post holder will be responsible for delivering a highly specialised clinical pharmacy service to patients accessing care within our East Intermediate Care Team in Ayrshires Health and Social Care Partnership. Experience will be gained working in a community based multidisciplinary team who support early discharge from hospital and prevent hospital admissions. This will involve conducting comprehensive medication reviews to ensure patients can be as safe and independent as possible with their own medication at home. You will need to possess a sound clinical knowledge, good IT skills, excellent communication skills, interpersonal skills, and problem solving skills, the ability to work autonomously and effectively as part of the multidisciplinary team.

We support development of our staff and applicants not currently holding a practice prescribing certificate will be supported to complete this. NHS Ayrshire & Arran has a large number of pharmacist prescribers working in secondary and primary care and to support patient-centred care in accordance with Achieving Excellence in Pharmaceutical Care.

Knowledge, training and/or experience required to do the job:

Essential

1. Masters of Pharmacy Degree orequivalent.
2. Scottish Hospital Pharmacy Vocational Training Scheme (or demonstrable evidence of clinical knowledge equivalent to that level).
3. Member of the General Pharmaceutical Council (includingevidenceofCPD)
4. Practice Certificate in Pharmacist Independent Prescribing(or working towards this qualification)

Desirable

5. Postgraduate Diploma in Clinical Pharmacy(or demonstrable evidence of clinical knowledge equivalent to that level)
6. Member of the Royal Pharmaceutical Society of Great Britain.

Experience

7. Relevant clinical pharmacy experience Good interpersonal skills
8. Full driving license
9. Computer literate with knowledge of information technology
10. Ability to work autonomously
11. Ability to work effectively as part of a team, in pharmacy and in the multidisciplinary environment

Knowledge

12. Required to demonstrate a highly specialised level of clinical pharmacy knowledge, skills, clinical reasoning and judgement.
13. Good knowledge and understanding of relevant standards and guidelines (National and local) Requires advanced arithmetic skills
14. Knowledge of community healthcare provision including GP and Community Pharmacy

Hours:

In accordance with AFC terms and conditions you may be required to work over 7 days and or an extended working day.

22.2 hours per week over 3 days (Wednesday - Friday).

Driving Licence/Car Ownership:

A car licence/car ownership is essential to this role.
