Job Title: Social Worker – Hospital Team

Rate: £35 per hour
Are you a passionate and dedicated social worker seeking a rewarding opportunity in a fast-paced hospital environment? An exciting opening has arisen at the Princess of Wales Hospital, Bridgend, for a social worker to join the dynamic Hospital Social Work Team within the Early Intervention & Prevention Hub.
What will your responsibilities be?
In this role, you will conduct comprehensive assessments to determine individuals’ needs in accordance with the principles of the Social Services and Wellbeing Act. You will develop and implement care plans in collaboration with multidisciplinary teams, and facilitate access to community resources. Additionally, you will conduct capacity assessments and address best interests, while advocating for individuals’ rights and services, ensuring ethical and legal compliance.

Qualifications and Experiences:
To be successful in this role, you must have Social Care Wales registration, be eligible to work in the UK, hold a full UK driving licence, possess post-qualified experience, and have a degree level or equivalent qualification in Social Work.
