About the Role:
Liquid Personnel is seeking an Adult Social Worker to join our client’s Community Wellbeing team based in Stoke-on-Trent. As an Adult Social Worker within the Community Wellbeing Team, you will support adults aged 18+ with care and support needs arising from physical disabilities, mental health conditions, learning disabilities, or age-related challenges. The role focuses on early intervention, person-centred planning, and strength-based approaches to promote independence and wellbeing in the community.
What will your responsibilities be?
In this role, you will
1. Carry out Care Act 2014 assessments to identify eligible needs and support planning.
2. Apply strengths-based and person-centred approaches to identify individual goals and outcomes.
3. Coordinate and review support packages, ensuring they are proportionate, cost-effective, and meet statutory requirements.
4. Complete safeguarding enquiries under Section 42 where concerns arise.
5. Undertake Mental Capacity Act assessments and support Best Interest decision-making where appropriate.
6. Work closely with health professionals, care providers, families, and community organisations to develop holistic support plans.
7. Promote reablement, prevention, and independence, making use of local resources and technology-enabled care.
8. Maintain accurate and timely case records using council systems such as Liquidlogic.
Qualifications & Experience:
To be successful in this role you must have,
9. Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
10. Registration: Must be registered with Social Work England.
11. Experience: Substantial experience working with vulnerable adults, including those with complex needs.
12. Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
13. Skills: Excellent assessment, analysis, and communication skills. Ability to work collaboratively in a multi-disciplinary setting.
14. Driving Licence: Full UK driving licence and access to a car is often required for community visits.
