Overview
As a Discharge to Assess Coordinator, you will help coordinate the discharge process for service users who need extra support when leaving hospital, ensuring they return to the most appropriate setting for a further assessment of their long-term care and support needs.
Based at the Transfer of Care Hub in Southmead Hospital, you will work with different partners to ensure service users are on the right pathway and move safely from hospital to home, community care or other services. You will work closely with senior staff to keep everyone informed about each service user’s discharge progress and make sure discharges happen smoothly and quickly.
What You Will Be Doing
* Maintain and update information systems and databases to ensure that service transactions are completed accurately.
* Coordinate the safe and timely discharge of service users from hospital to their appropriate pathway.
* Daily, communicate and work closely with social workers, hospital staff, care providers and other partners, ensuring they receive appropriate guidance about policies, procedures and processes while maintaining effective working relationships.
* Respond in a timely and appropriate manner to queries, complaints, and requests. Promptly relay issues that cannot be resolved to more senior team members.
* Involve in research, data collation and presenting information within the area of service specialism, including drafting documentation and clear, accessible findings.
* Provide a wide range of administrative tasks, including maintaining multiple spreadsheets to accurately record and monitor the work of the hospital team. Ensure records are accurate to inform decision making, and monitor and maintain budgets.
What We Need From You
* Educated to a minimum of 3 grade C GCSEs (or equivalent) including English and maths.
* Good understanding of working in the community in a social care or health setting, with the ability to understand individual needs.
* Experience of collating and distributing a range of information relating to service users.
* Good knowledge of Microsoft Excel and IT skills to use a client data system.
* Some experience in a relevant area of statutory or voluntary sector work in the health or social care field.
What You Need To Know
* Your salary is pro-rata to hours worked.
* Working pattern is predominantly 9am - 5pm, with a half day additional. The rota requires working on a Friday.
* This role is hybrid: mainly office based at Southmead Hospital, with opportunities to work from home.
