Overview
HRH are currently looking to employ a full-time Advanced Care Coordinator for the Enhanced Care Team to join our dynamic and evolving organisation.

Responsibilities
* Leading on care coordination for frail and palliative patients.
* Conducting home visits and clinical investigations (e.g. vitals, ECG, phlebotomy).
* Supporting patients and families with care pathways and end-of-life preparation.
* Coordinating and documenting MDT meetings and maintaining End of Life patient records.
* Supporting safeguarding duties, including maintaining registers and statutory compliance.
* Assisting with virtual and in-person care home ward rounds.
* Leading on allocated projects and supporting service improvements.
* Working as part of the Primary Care Network (PCN) multidisciplinary team under the direction of the PCN Clinical Director.

Qualifications
* Previous experience of working in a healthcare setting is essential.
