Social Worker – Adult Community Care Team
ASA Recruitment is recruiting on behalf of our client for an experienced Social Worker to join a locality-based Community Care Team in Denny.
Location: Denny
Hybrid Working: 50% home working / 50% office based following induction
Contract: Initially 3 months
Hours: 4 days per week (up to 30 hours)
Pay Rate: £22.00 – £35.00 per hour (dependent on experience)
Start Date: ASAP
About the Role
Working as part of a Community Care Team, you will support individuals, carers, and communities to achieve positive outcomes and maintain independence within their local communities.
Key Responsibilities:
* Undertake outcome-focused assessments, reviews, and risk assessments
* Support individuals to self-manage and access community resources
* Commission services through internal and external providers where required
* Maintain accurate records using Social Work Information Systems
* Manage a caseload of allocated service users and participate in duty services
* Carry out duties in line with Adult Support and Protection legislation
* Work collaboratively with NHS, home care, and third-sector partners
* Arrange appropriate equipment and adaptations to promote independence
* Participate in professional supervision and continuous professional development
* Contribute to innovative and person-centred ways of working
Essential Requirements:
* Minimum 2 years’ post-qualifying experience within an Adult Locality Team
* Degree in a relevant subject area
* DipSW, CSS, CQSW, Dip COT, BSc OT or equivalent qualification
* Registered with, or eligible to register with, the Scottish Social Services Council (SSSC) or Health and Care Professions Council (HCPC)
* PVG Membership (Adults) or willingness to obtain
* Excellent interpersonal and communication skills
* Strong organisational skills
* Sound knowledge of relevant Health and Social Care legislation
