Specialist Pharmacist - Heart Failure Rapid Titration

Nottinghamshire Healthcare Foundation NHS Trust has been successful in achieving funding to provide a Rapid Titration Service Evaluation across the county. This pioneering treatment has been successfully implemented in a number of locations across the country and this service evaluation places Nottinghamshire Healthcare in the enviable position of being one of the first community locations to complete Rapid Titration. The Service Evaluation will evidence the cost effectiveness of providing this treatment and the inevitable reduction it will have on waiting lists.

To support leading, delivering, developing and evaluating clinical pharmacy services to patients with cardiovascular diseases in accordance with the objectives set between the Speciality Clinical Lead, Directorate General Manager and Head of Service for Specialist Therapies. To be a role model in the field of cardiovascular diseases across the Nottinghamshire Integrated Care System.


Main duties of the job

To provide the prescribing pharmacy role in the Rapid Titration Heart Failure offering from the Nottinghamshire Healthcare Community Heart Failure Teams.

To act as a clinical role model for pharmacists and other members of the MDT involved in the care of patients with cardiovascular conditions.

Job responsibilities

Demonstrable ability to ensure safe, clinically effective and cost efficient use of drugs. This should include evidence of being able to:

* Deliver pharmaceutical services to acute and/or specialist medicine patients
* Deliver pharmaceutical services to cardiovascular patients
* Identify and promote best practice
* Ensure delivery of the clinical governance agenda in relation to drug use
* Motivate and inspire others
* Proactively develop clinical pharmacy services in line with local and national objectives
* Autonomous practitioner and experienced prescriber
* Experience of working with patients who have Heart Failure

A full current UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.


Person Specification


Qualifications

* MSc in Pharmacy
* Member of the General Pharmaceutical Council
* Non-Medical Prescriber
* UK driving license and access to a vehicle for work purposes
* Current membership of Royal Pharmaceutical Society
* Evidence of Leadership Development


Experience

* Clinical knowledge from degree and aspects of career to date
* Experience of new ways of working and extended pharmacy roles
* Extensive experience and clinical knowledge of HF prescribing
* Evidence based medicine practice
* Teaching experience
* Project management


Knowledge

* Clinical Pharmacy knowledge associated with a Pharmacy degree course and vocational experience
* Knowledge and experience of national HF prescribing and NICE guidance
* Personal safety e.g., lone working procedures


Skills

* Organisational / time management skills
* Tested problem solving, and decision-making skills
* IT literate
* Ability work as a team player
* Adaptable and flexible
* Work well under pressure and prioritise effectively
