Job Title: Social Worker – Adults Services (Hospital Discharge) Location: Preston, Lancashire (Royal Preston Hospital, Chorley Hospital & County Hall) Organisation: Lancashire County Council Pay Rate: £33.00 per hour Contract: Full-Time (37 hours per week) Service: Short Term Support Service (Prevention Service) Start Date: ASAP The Role: Lancashire County Council is seeking an experienced and motivated Social Worker to join the Adults Services team within the Short Term Support (Prevention) Service. This is a fast-paced and rewarding role focused on supporting timely and safe hospital discharges across Central Lancashire. You will play a key role in triaging referrals and planning discharges for residents, ensuring individuals receive the right support at the right time to promote independence and prevent readmission.
