We are looking for a Practice Nurse to work on a Saturday at one of our three sites in Ripon, Knaresborough and Harrogate to provide high quality evidence based nursing care to patients registered at one of our member practices.

Yorkshire Health Network is a federation of all 16 GP practices within the Harrogate and Rural area of the Humber and North Yorkshire ICB (integrated Care Board).
On behalf of the 16 GP practices, Yorkshire Health Network provides the Enhanced Access service at three locations, Ripon, Knaresborough and Harrogate for all patients registered with a GP in the Harrogate and Rural District Area.
Job responsibilities
Key Responsibilities
* Assess, plan, develop, implement and evaluate treatment programmes to promote health and well-being, and prevent adverse effects on health and well being
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
* Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
* Deliver opportunistic health promotion using opportunities such as NHS health checks
* Support patients to adopt health promotion strategies thatpromote patients to live healthily, and encourage principles of self-care
* Assess and care for patients presenting with uncomplicated wounds
* Support and advise women requesting information relating to family planning needs
* Support and manage health needs of women presenting for cervical cytology consultations
* Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health
* Promote and deliver evidence-based care for patients presenting with aural conditions
* Utilise communication skills to support patients to adhere to prescribed treatment regimens
* Anticipate barriers to communication and take action to improve communication
* Deliver care according to NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care
* Evaluate the patients response to health care provision and the effectiveness of care
* Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance
* Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients
* Ensure safe storage, rotation and disposal of drugs is undertaken
Person Specification
Skills
* Excellent communication skills (written and oral) including ability to communicate sensitive information clearly.
* Ability to work as a team member and autonomously.
* Experience of the web clinical systems (EMIS or SystmOne).
Experience
* Clinical experience working in a GP practice.
* Knowledge of needs of patients with long-term conditions.
* Knowledge of health promotion strategies.
Qualifications
* Accredited training in long term conditions and/or sexual health.
Other Requirements
* Availability to work during the operating hours for YHN services including afternoon, evenings and weekends.
