Job Description - Support Coordinator (251032)
Location: Teesside and Cleveland
Type: Permanent, Full or Part Time (37.5 or 15 hpw)
Pay: £13.21 per hour (Salary £25,838 per annum [pro rata for part time]) and great benefits including Health Cash Plan
Introduction: Want to be part of a team that cares and empowers you to make a difference? As a Support Coordinator, you'll support customers with complex needs to develop skills for independent living, making a positive impact every day.
Responsibilities:
* Creating support plans with customers and coordinating Support Workers.
* Working across services in Teesside with diverse customers.
* Assisting with daily activities like meals, shopping, budgeting, maintaining tenancies, and independent living.
* Conducting risk assessments, support planning, goal setting, and reviews.
* Engaging with customers through chats and catch-ups, and collaborating with colleagues.
What you bring:
* Passion for supporting customers to live their best lives.
* Experience in creating person-centred support plans.
* Experience in coordinating and assessing customer referrals.
* Ability to work independently, stay calm under pressure, and be resilient.
* Access to a vehicle insured for business use (mileage support provided).

Team: An outgoing, diverse, and supportive group led by Senior Client Service Manager Yvonne, who has been with Home Group since 2014.
Job Details:
