Main duties of the job
Working within your clinical and professional boundaries, you will work alongside the Durham Coast PCN Clinical Pharmacists to support clinical teams in all aspects of prescribing and medicines safety. This role promotes and supports improvements to maximise safe and best practice in prescribing and improve patient care quality. The Pharmacy Technician will be a point of contact for patient enquiries regarding prescriptions. The post holder will help patients get the best from their medicines through effective medicines reconciliation after discharge and other transfers of care, identifying patients potentially suitable for repeat dispensing, assisting in monitoring high-risk and high-cost drugs, and handling prescription queries within areas of competence. This also involves delivering key medicines optimisation outcomes against an ICB-set work plan, contributing to quality improvement and clinical audit, and supporting aspects of the Quality and Outcomes Framework, medicines safety, and antibiotic stewardship. You will also contribute towards practice financial stability through medicines optimisation and related QOF targets. The role includes training and developing practice-based administrative staff processing repeat prescriptions to support improved coordination and effective pharmacy administration in general practice, especially regarding repeat prescribing systems and processes.

We are looking to recruit an additional Pharmacy Technician to join our team at Durham Coast Primary Care Network which is formed of four member practices; East Durham Medical Group, Bevan Medical Group, Southdene Medical Centre and Wingate Medical Centre, together representing a population of over 33,000 patients.

Job responsibilities
Under the direction of our existing team ofPharmacists, we expect that you will be able to work independently and manageyour own workload. You will be self-motivated, and above all you will be passionate about providing the highest standards of care and treatment to patients in primary care.
Dueto the developing nature of the role and the needs and priorities ofindividuals requiring support, a high degree of flexibility is essential. Youwill need excellent communication skills, the ability to meet targets and to be able to liaise with patients, primary care staff, community pharmacists andother partners.
You willsupport the improvement of safety and quality of prescribing, optimise the bestuse of medicines whilst minimising risk and providing safe patient care.
In additionto the patient-facing aspect of your role, you will also be required to contribute towards practice financial and quality performance. You will supportthe Practice Pharmacists work in delivering medicines optimisation initiativeswithin GP practices as well as coding and supporting practices with their QOFregisters to ensure they are accurate.
The successful candidate will have excellent written and verbalcommunication skills, experience of influencing others and conducting clinicalmedication reviews.
It is a requirement of the role to be registered with theGeneral Pharmaceutical Society and have a BTEC/NVQ level 3 or equivalent inpharmaceutical sciences. The successful candidate would preferably have someexperience of working as a qualified, registered Pharmacy Technician.
Primary care experience would be an advantage but is notessential. Teamwork, flexibility, enthusiasm and the ability to prioritisetasks effectively are essential skills.
